Understanding what the medication is actually doing makes it far easier to use it well — and to keep the result once you're done.
It mimics a hormone you already make
GLP-1 (glucagon-like peptide-1) is a hormone your gut releases after you eat. It tells your brain you're full, slows how fast your stomach empties, and helps regulate blood sugar. Semaglutide and tirzepatide mimic that hormone — at higher levels and for much longer than your body produces it naturally.
Food noise gets quiet
Most patients describe it the same way: the constant mental chatter about food simply turns down. You feel satisfied sooner, stay satisfied longer, and stop grazing without white-knuckling it.
Insulin and blood sugar stabilize
GLP-1s improve insulin sensitivity and blunt post-meal glucose spikes. That's why energy steadies, cravings fade, and visceral fat — the metabolically dangerous kind — comes down first.
Tirzepatide adds a second receptor
Semaglutide works on the GLP-1 receptor. Tirzepatide is a dual GLP-1 / GIP agonist, which typically produces stronger appetite suppression and greater average weight loss. Dr. Tammy chooses based on your labs, history, and tolerance.
How you take it
One small weekly injection.
Nearly every patient tells us the same thing after their first dose: "That's it?"
Once a week, same day
Both semaglutide and tirzepatide are once-weekly subcutaneous injections. Pick a day — many patients choose Friday or Saturday so any early queasiness lands on a quieter day — and keep it. You can shift the day by up to 1–2 days if needed.
It's a tiny needle
A short insulin-style needle into the fat layer just under the skin. Most patients are surprised at how little they feel. We teach you in-office (or on a video visit) and you'll do your first dose with us walking you through it.
Store it cold
Keep vials or pens refrigerated (36–46°F). Don't freeze. Let the dose come to room temperature for a few minutes before injecting — cold medication stings more.
Timing is flexible
With or without food, morning or evening. Consistency matters far more than the clock.
Where to inject
Three easy spots — and rotate them.
GLP-1s are given subcutaneously, meaning into the fat layer just under the skin — not into muscle. Any of these three areas works.
- AbdomenThe most common site — at least 2 inches away from your navel. Easiest to reach and absorbs reliably.
- Outer thighThe fleshy front-outer part of the upper thigh. A good rotation option.
- Back of the upper armWorks well if someone can help, or if you can comfortably pinch the area.
- Rotate every weekMove the spot each dose — left abdomen, right abdomen, left thigh, right thigh — so tissue doesn't get lumpy or irritated. Avoid bruises, scars, moles, and tender areas.
Step by step
Wash your hands. Wipe the vial top or pen tip with an alcohol swab and let it dry.
If using a vial, draw exactly the number of units we prescribed — we write it down for you in units, not milliliters. Flick out air bubbles.
Swab your chosen spot with alcohol and let it air dry completely.
Gently pinch about an inch of skin and insert the needle straight in at 90 degrees.
Push the plunger steadily over 5–10 seconds, then wait a beat before withdrawing.
Needle straight into a sharps container — never the trash. We provide one and take it back when full.
You never have to figure this out alone. We demonstrate your first injection, send you home with written unit-by-unit dosing instructions, and you can call or text us with questions any week.
What to expect
Your first year, honestly.
- Week 1–2Starting dose
Lowest dose on purpose. Appetite starts to soften. Mild nausea in the first few days is common and usually fades within a week.
- Week 3–4Food noise drops
Most patients notice the quiet. This is when we lock in protein targets and hydration so you lose fat, not muscle.
- Month 2–3First titration
We step the dose up only if you need more effect and are tolerating the current dose. Slow titration is how we avoid the misery.
- Month 3–6Steady loss
0.5–2 lbs per week on average, with flat weeks in between. Labs and body composition re-checked. Hormones and thyroid adjusted as your metabolism shifts.
- Month 6–12Maintenance & off-ramp
Most patients lose 10–20% of body weight. We then taper toward the lowest effective dose or off entirely, with muscle, protein, and hormones holding the result.
Side effects
Most are manageable — if someone coaches you.
Side effects are the number one reason people quit a GLP-1 too early. Almost all of them respond to dose pacing, hydration, and eating differently.
Nausea
Smaller portions, eaten slowly. Skip greasy and very sweet foods. Ginger tea, peppermint, and bland protein (eggs, chicken, rice) help most. Stop eating the moment you feel full.
Constipation
Water, fiber at every meal, and magnesium citrate at night. This is the most common complaint and the most fixable.
Fatigue
Usually dehydration or simply under-eating. Hit your water and electrolyte target and eat at least one solid protein-rich meal even when you're not hungry.
Heartburn
Smaller meals, nothing late at night, and don't lie down for two hours after eating.
Muscle loss
The real risk nobody warns you about. Protein around 1g per pound of goal weight plus 2–3 strength sessions a week is non-negotiable — and we coach it.
